However, one condition -- gastroesophageal reflux disease -- stands out in its ties to obesity, say the authors of a new study. In the largest and most scientifically rigorous study to date, researchers at the University of Southern California were able to calculate that the effect of obesity on GERD is 13%.&lt;br /&gt;"Everyone knows that blood pressure and cholesterol levels are related to obesity, but the strength of the relationship with GERD is every bit as strong or perhaps even stronger," the authors of the study, Dr. Shahin Ayazi and Dr. Peter Crookes, told The Times. The study is published in this month's issue of the Journal of Gastrointestinal Surgery.&lt;br /&gt;As obesity rates have risen in the United States, so has GERD. One study suggested that GERD incidence increased 5% annually between 1992 and 2005. Almost 1 in 5 people have the disease. The volume and fat content of diet is linked to esophageal acid exposure as is high-calorie diets. The same dietary habits promote both obesity and GERD, the authors say.&lt;br /&gt;The study was conducted by examining almost 2,000 patients with GERD over 10 years. Researchers measured the amount of acid in the esophagus and used a probe to check the strength of the valve in the lower esophagus that normally protects against acid reflux. They were able to relate those measurements to the height and weight of their patients.&lt;br /&gt;The bad news about the study is that losing weight won't necessarily help with GERD unless someone is in the early stages of GERD and has mild reflux.&lt;br /&gt;"Heavier people have more reflux, and they have more damage to the lower esophageal sphincter," the authors said. "If you don't have a good sphincter, then you are likely to get worse reflux ... The sphincter is a muscle and in most parts of the body, when the muscle is damaged, it does not return."&lt;br /&gt;Of course, there are treatments for GERD. Then I read Melinda Beck’s July 22nd Health Journal in The Wall Street Journal titled “Baby crying? Doctors Say It May Be Acid-Reflux Disease.”&lt;br /&gt;&lt;br /&gt;Here’s the first paragraph of this article:&lt;br /&gt;&lt;br /&gt;Oliva Manganello was 1 month old when she started screaming, usually right after she nursed. Her family tried switching to formula, then different formulas, but nothing helped. Finally a pediatric gastroenterologist diagnosed gastroesophageal reflux disease (GERD) and put Olivia on Pepcid. “In two days, she was a completely different baby,” says her mother, Trina Chiara of Avon, Conn.&lt;br /&gt;&lt;br /&gt;When my mother read this article, her response was to wonder which of her grandchildren who cried all the time for the first few months might have had GERD.&lt;br /&gt;&lt;br /&gt;My husband accuses me of practicing medicine without a license. But I believe it’s important, as a do-gooder Scrooge, to know about common but often undiagnosed medical conditions in order to help others. &lt;br /&gt;&lt;br /&gt;For example, my older daughter called me from college when she worked part-time as an aide in a elementary school classroom to ask for information on allergies and asthma. Previous studies have shown that 50%-90% of people with asthma also suffer from gastric reflux, but until now the relationship between the two conditions was unclear.&lt;br /&gt;&lt;br /&gt;The study shows that inhaling small amounts of stomach acid back up into the esophagus and lungs, a hallmark of GERD, slowly produces changes in the immune system that may lead to the development of asthma.&lt;br /&gt;&lt;br /&gt;"This does not mean that everyone with GERD is going to develop asthma, by any means," researcher William Parker, assistant professor of surgery at Duke University Medical Center, says in a news release. "But it may mean that people with GERD may be more likely to develop asthma. If there is an upside to this, it is that developing GERD is something we can pretty much treat and control."&lt;br /&gt;&lt;br /&gt;Asthma-GERD Link Explained&lt;br /&gt;In the study, published in the European Journal of Clinical Investigation, researchers mimicked the effects of GERD in laboratory mice by inserting small amounts of gastric fluid into their lungs for eight weeks. They then compared how the immune systems of these mice responded to exposure to allergens, a key factor in the development of asthma, to the immune response of normal, healthy mice.&lt;br /&gt;&lt;br /&gt;The results showed that the GERD mice responded very differently to allergens. Specifically, they developed an immune response similar to that found in people with asthma by releasing a specific type of infection-fighting T-cell. Healthy mice responded in a more balanced manner releasing two types of immune cells.&lt;br /&gt;&lt;br /&gt;Researchers say rising rates of reflux and GERD may be driving increasing rates of asthma, but many of the risk factors behind this phenomenon may be modifiable.&lt;br /&gt;&lt;br /&gt;They say people who already have GERD can minimize gastric reflux and reduce their risk of developing asthma by following these tips:&lt;br /&gt;&lt;br /&gt;Eat smaller meals&lt;br /&gt;Eat several hours before going to bed&lt;br /&gt;Raise the head of the bed a few inches&lt;br /&gt;Maintain a healthy weight&lt;br /&gt;Limit fatty foods, coffee, tea, caffeine, and alcohol because they can relax the esophageal sphincter and make reflux more likely.&lt;br /&gt;"People should avoid the risk factors for GERD. The best drinks include: water, herbal teas or diluted non-citrus fruit juices. Please also note that some soft drinks (eg. cocoa, coffee, orange juice) are not helpful for heartburn as they can over-relax the LES, thus encouraging the backward flow of acid from the stomach.&lt;br /&gt;&lt;br /&gt;Eat More Fiber, Especially Soluble Fiber&lt;br /&gt;&lt;br /&gt;Constipation can lead to extra strain on abdominal muscles and increase the risk of heartburn. Relief of mild heartburn is typically achieved through diet and lifestyle changes, although medication (eg. antacids) is usually necessary if the heartburn is caused by gastroesophageal reflux disease (GERD).&lt;br /&gt;&lt;br /&gt;What is Heartburn? What Are The Symptoms?&lt;br /&gt;&lt;br /&gt;Heartburn is a digestive problem: it has nothing to do with your heart. It occurs when small amounts of stomach acid accidentally rise up the esophagus (the food canal running from the mouth to the stomach) causing symptoms like a burning pain in the chest which rises towards the throat. Sufferers may even taste the fluid in the back of the mouth, and this is called acid indigestion. In many cases the pain and burning sensation is relatively mild, but sometimes is so severe that patients think they are experiencing a heart attack.&lt;br /&gt;&lt;br /&gt;What Are The Causes Of Heartburn?&lt;br /&gt;&lt;br /&gt;In general, the immediate cause of heartburn is the acidic digestive juices which leave the stomach and rise up the esophagus. The acid burns the tissue of the esophagus causing inflammation and pain - a condition called esophagitis. Heartburn may also occur in pregnancy when the baby is pressing up against the underside of the diaphragm and squeezing the stomach. An occasional episode of acid-regurgitation or indigestion is quite normal. But persistent heartburn (eg. twice a week), is usually a sign of something more serious. The two most common underlying causes of heartburn symptoms are gastroesophageal reflux disease (GERD), and hiatal hernia.&lt;br /&gt;&lt;br /&gt;Gastroesophageal Reflux Disease&lt;br /&gt;&lt;br /&gt;GERD is characterized by a failure of the muscular valve, known as the lower esophageal sphincter (LES), which is located at the bottom of the esophagus. The function of this valve is to keep acid in the stomach and prevent it leaking back up the esophagus. When this valve doesn’t work properly, reflux (leak-back) and heartburn can occur. Factors that contribute to gastroesophageal reflux disease include obesity, pregnancy, smoking and, in particular, hiatal hernia.&lt;br /&gt;&lt;br /&gt;Hiatal Hernia&lt;br /&gt;&lt;br /&gt;Hiatus or hiatal hernia is a condition in which part of the stomach herniates (protrudes) upwards into the chest through the hiatus (opening) in the diaphragm normally only occupied by the esophagus. This puts extra pressure on the LES valve preventing it from closing properly. As a result, stomach acid leaks into the esophagus causing heartburn. The underlying cause of hiatal hernia is unknown. Contributory factors include: obesity, pregnancy, constipation, abdominal muscle strain, and smoking.&lt;br /&gt;&lt;br /&gt;Dietary Treatment For Heartburn&lt;br /&gt;&lt;br /&gt;In cases of persistent heartburn, dietary improvements are not sufficient. So as well as advocating a digestion-friendly diet, your doctor is likely to recommend one or more of the following types of medication. Antacids to neutralize the acid in your stomach; H2 blockers or proton pump inhibitors to reduce acid production; or prokinetics to strengthen the lower esophageal sphincter and expedite stomach-emptying. However, if heartburn symptoms are mild and occur only from time to time, usually you will be advised to make the following changes to your diet.&lt;br /&gt;&lt;br /&gt;If Overweight, Switch To A Healthy Weight Loss Diet&lt;br /&gt;&lt;br /&gt;Obesity, especially excess fat on the chest and abdomen, is a common contributory factor to GERD and hiatal hernia, and therefore to heartburn. So if you are obese (BMI &gt; 30), take steps to normalize your weight and thus reduce your risk or symptoms of heartburn. Choose a healthy calorie-controlled diet and customize it by following the eating tips below.&lt;br /&gt;&lt;br /&gt;Stop Eating Large Meals&lt;br /&gt;&lt;br /&gt;A big meal distends the stomach and causes a rise in the production of stomach acid. Both these factors lead to an increased risk of heartburn. So opt for 4-6 small meals or snacks, and allow a maximum of 3 hours between eating. It is usually on these nights that I am woken up by the dreaded reflux reaching my throat and sending me into a coughing fit that can last a good ½ hour or so.&lt;br /&gt;&lt;br /&gt;So what do I do? I listen to my body and try to stay away from late night junk food binges. Sure I’ll still have a treat every once in a while… but it won’t be late at night.&lt;br /&gt;&lt;br /&gt;I think if some people with acid reflux diet problems would simply listen to their bodies, and act accordingly… they would likely receive less acid reflux attacks. Again, this can vary between individuals and a more rigorous treatment may need to be discussed with your doctor.&lt;br /&gt;&lt;br /&gt;So which foods is more digestion friendly to those with bad heartburn?&lt;br /&gt;&lt;br /&gt;Again, everyone has different reactions to certain foods, so bear that in mind when you look at MY anti acid reflux diet below.&lt;br /&gt;&lt;br /&gt;Here is what I eat to help control my GERD symptoms. This type of diet also works well with the weight training that I do, as I need my protein intake a little higher than most people might.&lt;br /&gt;&lt;br /&gt;To me, this does not taste too much like an acid reflux bland diet... but it did take a couple of weeks of getting used to eating this way when I first started.&lt;br /&gt;&lt;br /&gt;Meal 1&lt;br /&gt;- 1/2 cup large flake oatmeal (not the instant stuff)&lt;br /&gt;- small banana&lt;br /&gt;- 6 egg whites prepared on the side&lt;br /&gt;- water&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Meal 2&lt;br /&gt;- sliced chicken breast on whole wheat or whole grain bread&lt;br /&gt;- a little fat free mayo&lt;br /&gt;- small apple&lt;br /&gt;- small salad (no tomatoes - too much acid) with low fat dressing&lt;br /&gt;- water&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;Meal 3&lt;br /&gt;- flank steak, chicken breast or very lean pork&lt;br /&gt;- boiled carrots, cabbage or greens beans&lt;br /&gt;- backed potato or yam&lt;br /&gt;- water&lt;br /&gt;&lt;br /&gt;NOW HERE'S MY TRICK!&lt;br /&gt;&lt;br /&gt;Because I need a little more protein from meat in my diet, I'll have a 4th meal about 1 to 1-1/2 hours before bedtime.&lt;br /&gt;&lt;br /&gt;What I have learned to really help keep my heartburn under control, is to avoid having any more starchy carbohydrates (i.e. bread, potatoes, cereal, etc.) after my 3rd meal above (which is usually at 6pm).&lt;br /&gt;&lt;br /&gt;By sticking with eating a piece of chicken breast and maybe some green beans for my last meal, I have found that I do not get acid reflux at night, nor am I woken up from severe heartburn.&lt;br /&gt;&lt;br /&gt;If I was to only eat 3 meals a day, I would probably have my last meal (#3 above) about 3 hours before going to bed. Natural Solutions magazine recommended taking a few tablespoons with meals to counter acid reflux.&lt;br /&gt;&lt;br /&gt;Meanwhile, the April issue of Health magazine listed honey-and-vinegar mixtures among its list of "health whoppers" that do nothing for arthritis pain.&lt;br /&gt;&lt;br /&gt;Although a daily cocktail of apple cider vinegar may sound like another celebrity trend or folk remedy, recent studies suggest vinegar might ease or prevent a variety of ailments.&lt;br /&gt;&lt;br /&gt;A Swedish study found that people who ate white bread with vinegar felt full up to two hours later, while those who ate just bread started losing their satiety after 30 minutes.&lt;br /&gt;&lt;br /&gt;In a study from Arizona State University, the blood sugar spikes of people with type 2 diabetes were 4 percent to 6 percent lower in the morning when participants took two tablespoons of apple cider vinegar before going to bed.&lt;br /&gt;&lt;br /&gt;In January, a report in the Journal of the American College of Cardiology recommended that an anti-inflammatory diet including vinegar "should be considered for the primary and secondary prevention of coronary artery disease and diabetes."&lt;br /&gt;&lt;br /&gt;These findings don't surprise or concern those who have preached the virtues of ACV - apple cider vinegar - for decades.&lt;br /&gt;&lt;br /&gt;Jerry Berube, 85, who lives in Montgomery, Ohio, with his wife, Patricia, says he hasn't had a cold or sore throat in 35 years. He's never battled weight gain or suffered from arthritis. He has no prescription medications.&lt;br /&gt;&lt;br /&gt;Berube partly credits his vitality to the drink he's sipped nearly every night since 1973: one tablespoon of organic apple cider vinegar mixed with one tablespoon of honey and 8 ounces of warm water.&lt;br /&gt;&lt;br /&gt;Even physicians who don't support ACV as a remedy see little harm in drinking moderate amounts diluted with water.&lt;br /&gt;&lt;br /&gt;"The placebo effect is very strong. I don't argue with that if it makes them feel better. But I can't promote it and say, 'Yes it's going to help you,'" says Dr. Debra Krummel, researcher with the University of Cincinnati's Department of Nutrition.&lt;br /&gt;&lt;br /&gt;Research from Arizona State University and the University of Lund in Sweden - the only groups known to be investigating ACV - has focused on ACV's potential for managing diabetes and hunger.&lt;br /&gt;&lt;br /&gt;"I was doing low-carb diets with diabetics. But I came across [ACV] and I thought, 'This could be easier than changing their entire diet,'" says Carol Johnston, chairman of the department of nutrition at Arizona State.&lt;br /&gt;&lt;br /&gt;In the study, acetic acid - found in any vinegar - controlled the blood sugar spikes that diabetics experience after a meal or first thing in the morning. Symptoms were only partially responsive to anti-asthma treatment including an inhaled corticosteroid and a leukotriene receptor antagonist.&lt;br /&gt;&lt;br /&gt;The patient's BMI was 27.8; she had gained over 10 kg in the previous two years. Typical symptoms of gastroesophageal reflux disease were not evident except for belch.&lt;br /&gt;&lt;br /&gt;Coughing worsened on eating and rising from bed. Although esophagography failed to disclose reflux esophagitis, esophageal pH monitoring revealed significant acid reflux.&lt;br /&gt;&lt;br /&gt;Asthma was considered well controlled. Treatment with the proton-pump inhibitor rabeprazole resulted in disappearance of cough.&lt;br /&gt;&lt;br /&gt;Frequency Scale for the Symptoms of Gastroesophageal reflux disease (FSSG) score, a questionnaire evaluating the symptoms of gastroesophageal reflux disease, was initially high but normalized after treatment. Using a validated GERD screening tool, 701 respondents were identified with GERD and the remaining were controls. Bonnie Dean, MPH, PhD, of Cerner LifeSciences, Ronnie Fass, MD of the University of Arizona and their research team found that sleep impairment was more common among people with GERD (41.9 percent) than those without GERD (19.4 percent). Researchers found that 49.5 percent of respondents with nighttime GERD reported sleeping poorly often or most of the time, compared to 36.7 percent of people with daytime GERD. &lt;br&gt;&lt;br&gt;Using the survey, researchers also assessed sleep impairment among patients experiencing frequent nighttime atypical manifestations of GERD. In this case, Dr. Dean and her colleagues evaluated the subgroup of respondents with GERD, as identified using the validated GERD screener. They found that atypical manifestations or symptoms of GERD (i.e. coughing, sore throat, snoring, wheezing, choking, and chest pain) were common among those with acid reflux. Of GERD patients, 74 percent had at least one nighttime atypical manifestation. For almost every daytime and nighttime atypical manifestation assessed, more than 20 percent of GERD patients reported their occurrence as frequent (more than 2 days or nights per week). Researchers also found that sleep impairment was more common among GERD patients with atypical manifestations compared to GERD patients with only typical or classic symptoms such as heartburn and acid regurgitation. The mechanism, new in the United States, has been proven safe and effective in patients treated throughout Europe for gastroesophageal reflux disease (GERD), also known as heartburn.&lt;br&gt;&lt;br&gt;&amp;quot;The medical device offers a treatment for patients who suffer from an advanced degree of GERD and, until now, would have been candidates for surgery,&amp;quot; said Dr. Scott Melvin, division director, general and gastrointestinal surgery, and director, Center for Minimally Invasive Surgery at the OSU Medical Center.&lt;br&gt;&lt;br&gt;Heartburn is a condition in which acid refluxes from the stomach into the esophagus. The burning sensation felt in the chest can last for countless hours, resulting from inflammation, bleeding or ulcers in the esophagus. GERD is known to be associated with an increased risk of cancer of the esophagus.&lt;br&gt;&lt;br&gt;Almost 30 million Americans are diagnosed with chronic reflux disease that requires medication for relief of symptoms. It is estimated that millions of dollars are spent on over-the-counter and prescription medications for treatment of the condition.&lt;br&gt;&lt;br&gt;The incisionless surgery allows reconstruction of the one-way valve at the top of the stomach, when the valve is defective. The new tubular device is introduced to the body totally through the mouth, and is then advanced down the esophagus into the stomach. During the procedure, the operation is viewed through a small fiberoptic camera located within the tubular surgical tools. Patients are normally in the hospital overnight and, after the procedure, are symptom free.&lt;br&gt;&lt;br&gt;&amp;quot;Throughout medical history, we have gone from a very painful surgery, to a better tolerated surgery and, now, to a potentially pain-free incisionless procedure. Foxx-Orenstein, DO, FACG, &amp;quot;Heartburn medications to treat acid reflux during pregnancy should be balanced to alleviate the mother&amp;#39;s symptoms of heartburn, while protecting the developing fetus.&amp;quot;&lt;p&gt;Based on a review of published scientific clinical studies (in animals and humans) on the safety of heartburn medications during pregnancy, researchers conclude there are certain drugs that are considered safe for use in pregnancy and those which should be avoided.&lt;p&gt;Antacids are one of the most common over-the-counter medications to treat heartburn. As with any drug, antacids should be used cautiously during pregnancy.&lt;p&gt;Antacids &lt;p&gt;Antacids containing aluminum, calcium, or magnesium are considered safe and effective in treating the heartburn of pregnancy. &lt;br&gt;Magnesium-containing antacids should be avoided during the last trimester of pregnancy because it could interfere with uterine contractions during labor. &lt;br&gt;Avoid antacids containing sodium bicarbonate. Sodium bicarbonate could cause metabolic alkalosis and increase the potential of fluid overload in both the fetus and mother. &lt;br&gt;Histamine-type II (H-2) Receptor Antagonists &lt;br&gt;While limited data exists in humans on the safety of histamine-type II (H-2) receptor antagonists, ranitidine (Zantac?) is the only H-2 antagonist, which has been studied specifically during pregnancy.&lt;p&gt;In a double-blind, placebo controlled, triple crossover study, ranitidine (Zantac?) taken once or twice daily in pregnant heartburn patients not responding to antacids and lifestyle modification, was found to be more effective than placebo in reducing the symptoms of heartburn and acid regurgitation. No adverse effects on the fetus were reported. The following tips can help reduce heartburn discomfort:&lt;p&gt;Avoid eating late at night or before retiring to bed. Common heartburn triggers include greasy or spicy food, chocolate, peppermint, tomato sauces, caffeine, carbonated drinks, and citrus fruits. &lt;br&gt;Wear loose-fitting clothes. Clothes that fit tightly around your waist put pressure on your abdomen and the lower esophageal sphincter. &lt;br&gt;Eat smaller meals. Overfilling the stomach can result in acid reflux and heartburn. &lt;br&gt;Don&amp;#39;t lie down after eating. Wait at least 3 hours after eating before going to bed. When you lie down, it&amp;#39;s easier for stomach contents (including acid) to back up into the esophagus, particularly when you go to bed with a full stomach. &lt;br&gt;Raise the head of the bed 4 to 6 inches. Once it begins, it usually is life&amp;#226;&amp;quot;long. If there is injury to the lining of the esophagus (esophagitis), this also is a chronic condition. Moreover, after the esophagus has healed with treatment and treatment is stopped, the injury will return in most patients within a few months. Once treatment for GERD is begun, therefore, it usually will need to be continued indefinitely although it is argued that in some patients with intermittent symptoms and no esophagitis, treatment can be intermittent and done only during symptomatic periods.&lt;p&gt;In fact, the reflux of the stomach&amp;#39;s liquid contents into the esophagus occurs in most normal individuals. One study found that reflux occurs as frequently in normal individuals as in patients with GERD. In patients with GERD, however, the refluxed liquid contains acid more often, and the acid remains in the esophagus longer. It has also been found that liquid refluxes to a higher level in the esophagus in patients with GERD than normal individuals. &lt;p&gt;As is often the case, the body has ways (mechanisms) to protect itself from the harmful effects of reflux and acid. For example, most reflux occurs during the day when individuals are upright. In the upright position, the refluxed liquid is more likely to flow back down into the stomach due to the effect of gravity. In addition, while individuals are awake, they repeatedly swallow, whether or not there is reflux. Each swallow carries any refluxed liquid back into the stomach. Finally, the salivary glands in the mouth produce saliva, which contains bicarbonate. With each swallow, bicarbonate&amp;#226;&amp;quot;containing saliva travels down the esophagus. The bicarbonate neutralizes the small amount of acid that remains in the esophagus after gravity and swallowing have removed most of the liquid.&lt;p&gt;Gravity, swallowing, and saliva are important protective mechanisms for the esophagus, but they are effective only when individuals are in the upright position. At night during sleep, gravity is not in effect, swallowing stops, and the secretion of saliva is reduced. Therefore, reflux that occurs at night is more likely to result in acid remaining in the esophagus longer and causing greater damage to the esophagus.&lt;p&gt;Certain conditions make a person susceptible to GERD. For example, GERD can be a serious problem during pregnancy. The elevated hormone levels of pregnancy probably cause reflux by lowering the pressure in the lower esophageal sphincter (see below). At the same time, the growing fetus increases the pressure in the abdomen. Both of these effects would be expected to increase reflux.
